I, hereby freely consent to participate in the recommended treatment programme (based on my medical history, diagnosis, symptoms, and assessment results) delivered by those authorized in this centre I am aware and understand that Physiotherapy treatment is generally an effective and safe form treatment however like an treatment there are benefits and risks and the response individual treatment varies and cannot always be predicted as each person’s condition is unique to him or her.I do not expect the Physiotherapist to anticipate all possible risks and complications I wish to rely on the physiotherapist to exercise proper judgement during the course treatment to make decisions based upon my best interest. I accept the fact that that there is no guarantee of the effectiveness of the treatment and hence I say that I will not hold the ADAM WYLIE PHYSIO REHAB CENTRE and the Doctors/support staff working in the ADAMS WYLIE PHYSIO REHAB CENTRE responsible and/or liable if the Physiotherapy treatment given to me does not give the desired results or if I suffer complications whilst undergoing treatment.
